INSTRUCTIONS

1. Application Form must be filled in Block Letters.
2. I PUC application form must be filled according to candidate’s SSLC/ICSE/CBSE
   Hall ticket (i.e Candidate’s name, father’s name, mother’s name and date of birth).
3. At the time of submission kindly enclose the following only:
   « Provisional marks card signed by the school Principal
   « Caste certificate if the candidate belongs to SC/ST/Cat I, IIA, IIB, IIIA, IIIB caste groups.
   « Candidate’s SSLC/ICSE/CBSE Hall ticket Xerox copy
4. Application incomplete in any respect will automatically become invalid.
